    Please complete prior to initial consultation. The main purpose of a medical history questionnaire is to find out about a client’s health history and get an idea of his/her health.

  • Informed consent

    Is a process by which an informed client consents to participate in the massage treatment. It allows the therapist to involve the client in choices related to his wellness.

things to consider when booking your appointment

 

no work when experiencing

  • edema- If due to bacterial or viral infection, pitted edema (indicates tissue fragility), lymphatic obstruction due to parasites, deep vein thrombosis, or cardiac, liver, kidney disease

  • feels physically ill or nauseated (start of a cold), severe pain, fever, has been seriously injured recently

  • appendicitis, rashes (caused by systemic disease such as lupus or Lyme disease), hives (General contraindication in acute stage), skin cancer (If metastasizing)



limited work when experiencing

  • acute inflammation (signs are redness, heat, swelling, pain), rheumatoid arthritis. over acutely inflamed joints; avoid traction of joints and spine, Phlebitis, locally inflamed tissue.

  • skin conditions-acne, rashes, boils, athlete’s foot, ringworm, herpes simplex (cold sores), impetigo, hives (in subacute stage), allergies

  • burns, recent fracture, acute or unset broken bones, sprains/strains (in acute phase)

  • severe varicose veins, cardiac arrhythmias, blood poisoning (inflammation of the lymphatic vessels), swollen glands (lymph nodes), bruising
